The Osman Approach is quite simple: conduct spine surgery in the LEAST INVASIVE manner possible! It was in 1993 that Dr. Osman began working on his approaches, and they have since evolved, starting in 1994 with the publication of his cadaver study – Posterolateral Arthroscopic Discectomies of the Thoracic and Lumbar Spine. This was followed by the first endoscopic thoracic discectomy and fusion, as an outpatient procedure in 1995 – the patient, a radiologist from Hawaii, gave a live television interview on Channel 3 News, on the 4th day after his surgery, fully dressed in his suit, and while walking in his hotel lobby! Since then, Dr. Osman has continually developed new endoscopic spine techniques over the years (see below). As time goes on, the approaches get more advanced through small incisions and less invasive routes. By using an endoscopically-assisted approach, Dr. Osman is able to treat spine patients’ conditions in ways that not too long ago were never thought to be possible. As a global thought leader in this discipline, Dr. Osman is constantly pioneering unique techniques and ways in which to make spine patients’ lives better.

First Endoscopic Unilateral Bi-Portal Thoracic Discectomy and Interbody Fusion, 1995.
This procedure was performed by Dr. Said G Osman, M.D. in 1995 and avoided opening the chest or spinal canal by removing bone. Additionally, the procedure was performed through 6-millimeter portals – one for the endoscope and the other for instruments, thereby allowing more freedom of instrument choice and instrument manipulation. The procedure involved fluoroscopic identification of the appropriate spinal segment, Instrumentation of the spine, removal of the offending disc, decompressing the spinal cord, and interbody fusion. By avoiding entry into the chest cavity, Dr. Osman avoided intensive care treatment for the patient (this would have been necessary for a trans-thoracic approach to re-expand the lung). Dr. Osman has been using this approach for 21 years now with much, much less post-operative complications seen following other more invasive approaches.
First Endoscopic Lumbar Foraminoplasty Using Unilateral, Bi-Portal Approach and Automated Tools – 1996.
The video shows the first ever endoscopic lumbar foraminoplasty, performed through unilateral, bi-portal approach, using multiple tools, including automated shavers to remove soft tissue, automated burs to remove bone encroaching on the foraminal canal, curettes, probes and other ancillary tools. The patient in this video was a 72-year-old retired surgeon who had fallen off the roof of his garage while shoveling snow, 12 months prior to his surgery! He fractured the L1 vertebra with displaced fragment which occluded the L1-2 foramen on the left. He had severe, unrelenting left groin pain. It was before the era of Kyphoplasty and the only surgical option offered was fusion through the chest and abdomen (thoracoabdominal approach). Because of his age and potential trauma and complications, he opted to be the first patient for endoscopic foraminoplasty using unilateral, bi-portal approach and automated tools. The surgery took less than one hour. He woke up with tears of joy streaming down his face because, for the first time in 12 months, he woke up without left groin pain!
